To subscribe to the T-Mobile network services visit one of our sales points with the required documents listed below:

For a customer who is already an T-Mobile subscriber the required documents are:
  • Identity card and authority to sign an agreement (if necessary)
  • Original company registration certificate and a written declaration stating that all data (NIP, REGON) is up-to-date

Additionally, a confirmation, which can be acquired from the sales point manager, of no overdue payments for other phone services (subscription account included) is required.

When entering into an agreement, the customer pays only for the handset, if he or she is buying a new phone with the activation service. If the customer is buying the activation only, there is no fee.

For your convenience there are two sets of required documents to choose from:

Set I

  1. Identity card or passport of individual making the purchase,
  2. Function attestation
In case of plenipotentiary the following are required:

Original document of plenipotentiary appointment with Embassy's seal

Set II - the customer is already an T-Mobile network subscriber

  1. Letter from head of organization entering into a contract with the following information enclosed:
    1. instructions to enter into a selected number of contracts with the T-Mobile network
    2. personal information necessary to enter into a contract (name, surname, ID number, date of birth)
    3. seal and signature of head of organization
    4. date and place when document was drawn up
    5. letter composed on organization's letterhead - recommended

No additional documents are required




Representatives of foreign companies
Individuals entitled to enter into a contract Required documents
A company representative specified within the license or an appointed plenipotentiary
  1. Identity card of the individual making the purchase;
  2. original economic activity license issued by Ministry;

In case of plenipotentiary the following are required:

Original document of plenipotentiary appointment - unless the plenipotentiary produces a document stating that he/she represents the company;
